<dc:description>Objective: To investigate the clinical laboratory characteristics and gene mutation features of dual driver gene positivity in patients with Ph-negative myeloproliferative neoplasms (MPN).Methods: A retrospective analysis was conducted on the clinical data and genetic test results of 203 newly diagnosed Ph-negative MPN patients, including 194 with single driver gene positivity and 9 with dual driver gene positivity. High-throughput sequencing was used to detect JAK2, CALR, and MPL gene mutations, comparing the clinical characteristics and gene mutation profiles of the two groups of patients. Results: The incidence of dual driver gene positivity was 4.4% (9/203), mainly comprising JAK2 combined with CALR (4 cases) and JAK2 combined with MPL (4 cases). Compared to the single driver gene positive group, the dual driver gene positive group had a significantly higher risk of bleeding [4.1% (8/194) vs. 33.3% (3/9), P=0.008] and a higher proportion of non-classical mutations [3.6% (7/194) vs. 44.4% (4/9), P=0.001]. There was no statistical difference between the two groups in terms of age, thrombosis incidence, splenomegaly, and routine blood test indicators. During the follow-up period, 1 case in the dual driver gene positive group died due to cerebrovascular disease, with no leukemia transformation or disease-related deaths in the remaining cases. Conclusion: The patients of Ph-negative MPN with dual driver gene positivity mayhave a higher risk of bleeding and a more complex gene mutation profile.</dc:description>
