<dc:description>This dataset contains survey data from Sichuan Province, China, collected as part of the national chronic disease prevention and control capacity assessment. It includes two cross-sectional survey waves conducted in 2013 and 2017.The survey employed stratified random sampling covering all prefecture-level regions of Sichuan Province. The surveyed facilities included two types of primary healthcare institutions: Community Health Service Centers (CHSCs, urban) and Township Health Centers (THCs, rural).A total of 439 valid questionnaires were collected in 2013, and 309 valid questionnaires were collected in 2017. The survey covers multiple dimensions, including basic facility information, human resource allocation, fiscal investment, chronic disease prevention and control service delivery, and service quality indicators. This dataset can be used to assess temporal trends in grassroots chronic disease prevention and control capacity, analyze urban-rural disparities, identify key factors influencing service capacity, and provide empirical evidence for health policy-making and resource allocation optimization. Research based on this dataset employed Latent Class Analysis (LCA) to classify facility service profiles and explored their associations with staffing levels, fiscal investment, and regional economic conditions.Notes:All data have been de-identified. Institution names, specific addresses, and other identifiable information have been removed to protect the privacy of participating facilities.Data format: CSV, compatible with Excel, R, SPSS, Stata, and other statistical software.Variable names in the dataset correspond directly to the questions in the survey questionnaire. The specific meaning, coding options, and value ranges of each variable are fully consistent with the survey questionnaire. Please refer to the questionnaire files (questionnaire_2013.pdf / questionnaire_2017.pdf) for the exact wording and response options of each question. Each row in the dataset represents one primary healthcare institution.Data Collection Method:Survey method: Self-administered structured institutional questionnaireSampling method: Stratified random samplingSurvey years: 2013, 2017</dc:description>
