<dc:title>Clinical Investigation of Early Vascular Endothelial Injury in Patients with Asymptomatic Hyperuricemia</dc:title>

<dc:description>Objective: To explore the clinical characteristics of early endothelial injury in asymptomatic hyperuricemia patients, analyze the mediating role of immune inflammatory factors in uric acid induced endothelial injury, and provide clinical evidence for early prevention and control of cardiovascular risk in asymptomatic hyperuricemia. Method: 120 asymptomatic patients with hyperuricemia and 120 healthy individuals who underwent physical examinations during the same period were included. Ultrasound was used to detect brachial artery blood flow mediated vasodilation function (FMD) to evaluate endothelial function, and serum endothelial injury markers and levels of immune inflammatory factors were detected; By using a multiple linear regression model to analyze the independent influencing factors of early endothelial injury, a mediation effect model was constructed using Baron Kenny method combined with Sobel test to verify the mediating effect of interleukin-6 (IL-6). The results showed that the levels of FMD in the asymptomatic hyperuricemia group were significantly lower than those in the control group, while the levels of endothelin-1, von Willebrand factor, soluble vascular cell adhesion molecule-1, IL-6, tumor necrosis factor - α and other inflammatory factors were significantly higher than those in the control group (all P<0.001); After controlling for confounding factors, blood uric acid was found to be an independent risk factor for endothelial injury (β=-0.013, P<0.001); IL-6 plays a partial mediating role between blood uric acid and endothelial injury, with the mediating effect accounting for 35.5% of the total effect. The Sobel test showed statistically significant differences (Z=-3.220, P=0.001). Conclusion: In the stage of asymptomatic hyperuricemia, clear early damage to the vascular endothelium has appeared, and blood uric acid can exacerbate endothelial function damage by inducing chronic low-grade immune inflammatory response mediated by IL-6; The combined detection of blood uric acid and inflammatory factors can be used for early screening and risk assessment of vascular injury in high-risk populations. </dc:description>
