<dc:title>Resistance Evolution and Dissemination Mechanisms of Vaccine-Escaped S. pneumoniae in China: A Genomic Epidemiological Study</dc:title>

<dc:description>Objective To delineate serotype distribution, antibiotic resistance profiles, and evolutionary mechanisms of Streptococcus pneumoniae immune escape strains in the post-PCV13 era in China, providing insights for vaccine optimization and resistance control. Methods Forty-eight clinical isolates of S. pneumoniae (2010–2024) meeting inclusion criteria were selected from the Global Pneumococcal Sequencing (GPS) database. Integrated analyses were performed using the IPGA platform (National Microbiology Data Center), including serotyping (SPNserotype v2.0), resistance gene annotation (ABRicate v1.0.1), clonal complex analysis (PopPUNK v2.4.1), and recombination hotspot detection (PhiPack v3.5.0).Results Dominant serotypes were type 3 (39.6%, 19/48), 19A (31.3%, 15/48), and 19F (14.6%, 7/48), with PCV13 escape strains accounting for 81.3% (39/48).ST6011 (27.1%) and ST320 (22.9%) were predominant MLST types. The GPSC1-ST320 clone co-harbored ermB (macrolide resistance) and tetM (tetracycline resistance) genes (co-occurrence: 85.7%). Penicillin high-level resistance (MIC ≥4 μg/mL) significantly correlated with pbp2x mutations (P<0.001).An open pan-genome structure was observed (core gene proportion: 43.2%). High-frequency recombination occurred in the pspC-nanA region (μ=9.2 events/Mb, 4.7× genomic average). Unique genes in outlier strain GPS_HK_464 (n=312) were enriched in mobile genetic elements (mefA-msrD efflux module). Conclusion Serotype 3/19A/19F dominates PCV13 escape strains in China. The ST6011-ST320 clonal complex achieves evolutionary advantage through mobile element-mediated resistance synergy (ermB/tetM) and high-frequency recombination.</dc:description>
