<dc:description>Objective To evaluate Chinese caregivers' willingness to accept monoclonal antibodies (mAbs) against respiratory syncytial virus (RSV) for infants and its determinants. Methods A hospital-based survey was conducted from December 2024 to January 2025 to collect data on participants' demographic characteristics, awareness of RSV infection and anti-RSV mAbs, and willingness to prevent RSV infection using mAbs in infants. Multivariable conditional logistic regression analysis was performed using stepwise backward selection based on conditional likelihood ratio criteria and informed by clinical judgement, to identify factors influencing caregivers' decisions. For caregivers unwilling to accept the mAbs, brief information interventions were delivered, and changes in willingness and reasons for refusal were assessed to evaluate the intervention's impact. Results Among 594 valid responses, 63.0% expressed willingness to use mAbs for RSV infection prevention, 65.0% had high awareness regarding the severity of RSV infection, 62.6% recognized the benefits of anti-RSV mAbs, and 50.5% were aware of anti-RSV mAbs accessibility; however, only 15.5% recognized infants' susceptibility to RSV infection. Respondents who had vaccinated or planned to vaccinate family members at their own expense to prevent respiratory tract infections (OR: 1.740 [1.172-2.584], P=0.006), who had higher awareness of infants' susceptibility to RSV infection (OR: 2.856 [1.699-4.799], P<0.001), and who had higher awareness of anti-RSV mAbs accessibility (OR: 3.240 [1.669-5.259], P<0.001) were more likely to prevent RSV infection using mAbs. If anti-RSV mAbs were provided free of charge, 89.3% of caregivers would be willing to accept them. Additionally, 55.9% (123/220) of initial refusers changed their willingness after a brief information intervention. Conclusions Improving caregivers' willingness to use mAbs for RSV prevention in infants depends on continuing to cultivate the habit of out-of-pocket vaccination against respiratory infections, strengthening education regarding infant susceptibility to RSV and anti-RSV mAbs accessibility, and providing financial support, such as including anti-RSV mAbs in the national immunization program.</dc:description>
